August 13, 1961 – November 19, 2020
Tammy Lynn Choate, 59, of Ponca City died on November 19, 2020 at Alliance Health in Ponca City, surrounded by her loving family. A funeral service will be held at 10:00 am Wednesday, November 25, 2020 at the Trout Funeral Home Chapel, with Pastor Alan Jones officiating. Interment will immediately follow at Resthaven Memorial Park.
Tammy was born on August 13, 1961, in Joplin, Missouri, to Clarence and Clara (Surine) Hawkins. She graduated from Broken Arrow High School in 1980. She was an adored job coach at the Opportunity Center in Ponca City for the last 15 years.
Tammy enjoyed spending time with her family, crafting and bowling with her many friends and loving husband Harold. You never had to wonder what her average was for the night, she’d make sure everybody knew. That bowling score was the only average thing about Tammy. She was an extraordinary, loving person which was especially shown with her many years of work at the Opportunity Center. She loved her clients and they loved her just as much. They were a huge part of her family. She had a very special place in her heart for her children and grandchildren, she loved them all unconditionally. Tammy shared a special bond with her mother-in-law, Nelva, or Bud as they liked to call each other. She will be missed by the many who were blessed to be loved by her.
She is survived by her husband, Harold Choate, of the home; sons Joshua Floyd (Jessi) of Chandler, OK, Gerald Floyd of Ponca City, OK; Tony Choate; Mark Choate (Tammi); Brian Choate; Matthew Choate (Tiara); Michael Choate (Shayna); daughter Tanna Floyd (Mark) of Blackwell, OK. Brothers Everett Hawkins of Shidler, OK and Bill Hawkins (Shelly) of Pryor, OK and sister Janell Rhynard (Steven) of Braman, OK. Numerous nieces and nephews, 30 grandchildren and 2 great grandchildren.
Tammy was preceded in death by her father, Clarence Hawkins and mother, Clara (Surine) Hawkins.
